Community Connections (CC)


Community Connections provides community access support services to residents of Logan City and surrounds
who may have any of the following disabilities alone or in combination:

  • moderate to severe intellectual disability;
  • sensory disability;
  • acquired brain injury;
  • physical disability; and
  • psychiatric disability.

Our assessment for services is also based on the availability of CC resources (for example transport,
staff and facilities) and level of need.

Who can access this service?

Clients 16 years of age and older and no longer undertaking secondary studies are eligible
for assessment for CC services.
Level of need is based on:
  • risk of isolation from peer group and social activities
  • risk of loss of independent living skills

Via case-managed group and individual services, we offer clients:

  • opportunities for social and recreational involvement
  • opportunities to maintain and improve skills
  • links to local community activities and services
  • assistance to maintain their independence
Priority will be given to those clients with the least support in place. 
Service planning and review is open to clients, carers and advocates at all times.
