What do we do?
The Lifelong Planning Support Officer Strategy aims to provide a model of prevention and early intervention support at the high risk, transition points in the lives of community members with a disability. The YFS LPSO does this in two ways:
 
Community Development:
Active participation in a number of community disability and service provider networks.
Planning and participation in activities to help build the
community’s capacity to support all community members. 
 
Information and Referral:
This service provides one off or short term assistance to help community members with a disability, their families or carers to access support services. This may include helping people to identify the most appropriate support, providing information on available services or contacting service agencies on behalf of a community member.
 
The service is unable to provide comprehensive case management services. Community members will be referred to appropriate agencies for follow up support if required.
 
Who can use this service?
Anyone aged 0-65 with a disability or their families or carers can access this service. 
 
Clients do not have to meet DSQ eligibility criteria. 
 
Access to the service is incident based. 
Clients may re-access the service at any time. 
 
How can I use this service?
The Lifelong Planning Support Officer hours are:
Monday, Wednesday, Thursday 8:30am-4:30pm
