Home and Community Care (HACC)
What do we do?
YFS HACC provides services to foster independence and encourage active participation in the community.
A formal assessment is made to determine your eligibility and needs, and our ability to respond. If YFS
HACC cannot meet your needs, we can assist with referrals to other services.YFS HACC provides
two types of HACC services:
YFS HACC offer planned group activities for people wanting to socialise, learn and develop their skills.
A small cost applies to some of these activities.
Home Care
Support for individuals to promote independence, confidence and skills development.
This can include: assistance to plan your budget, getting to appointments, assistance to plan your
child-care or access to other community services.
Who is able to access the service?
Services are available to people (under 65 years) with a moderate to severe disability.
