Midwives know the miracle of birth is not just the making of babies, but the making of parents.  

The Maternity and Child Health Hub (Community Connector) (MaCHHub) program improves health outcomes for pregnant people, new parents and their babies through strong, ongoing support and continuity of midwifery care.   

The same midwife provides care throughout pregnancy and for up to six weeks after birth, helping build trust and continuity of care. At the same time, a Community Connector can connect you to relevant local and YFS services that will help you as you become a parent. 

The Hub is based on an innovative, best practice model developed by the community for the community. It is an extension of Logan Hospital services.

What to expect

If your application is accepted, you will be allocated a YFS MaCHHub midwife. You will also be contacted by the Community Connector who will discuss your needs with you and find out how the MaCHHub can best support you, your family and your unborn baby.

Midwives help with medical needs and see you monthly until you are close to delivery. Our Community Connector can help you with other needs (like connection to housing, financial and counselling support) and will work with you to access these in the best way for you and your family.

The service will support you up until six weeks after your baby is born, after that you will be offered ongoing support from Children’s Health Queensland who can visit you and your baby in your home to check that everything is progressing well and answer any questions you may have. All support is voluntary and free.

This service supports people in the Logan Local Government Area (LGA).

How to access this service

You can self-refer to Logan Hospital Midwifery Group Practice (MGP), in person at the YFS Maternity and Child Health Hub or online via the Self referral form.

Information for professionals & referrers

This program supports pregnant people with complex social needs. Pregnant people currently working with a YFS program can be directly referred into the YFS Maternity and Child Health Hub (MaCHHub).  

A YFS referral should be accompanied by a GP referral into the Metro South Health MaCHHub where possible, but pregnant people can still be accepted into the program without a GP referral.
